How do you other digital painters handle perspective when drawing directly on the computer? Often the vanishing points are off the page. On a table you can physically chart out where the vanishing points are are and use a ruler. While you can draw straight lines on a computer, it's hard to chart points in mid-air next to your monitor. The only solid solution I can think of offhand is to add canvas to the sides of the image (and top or bottom for 3-point), and extend your horizon line in this way.
Any other solutions?

When I'm producing something from scratch on a computer I'll just do the perspective lines in a 3D package. Sometimes I'll go as far as to block out a scene in 3D using basic geometry then experimenting with different lens widths to see what different effects it'll produce. Then you just render out the image and whack it on a layer in Photoshop, a basic starting point to sketch over.
