It's quite easy:
1. Get an empty image with a white background
2. Having the brush in b/w is the best. Put the texture you want to use in the image, clean it up - means, make some natural contours for the brush. Everything that's white will be transparent.
3. Put a selection-box around your brush-to-be.
4. The brush-palette has a menu on the right side - click on the little arrow/triangle thingie on the top right corner and it will open up. Choose 'Define Brush'
5. Voila - the new brush should pop up in the brush menu.
For any more questions refer to the manual (which you have, don't you?).
If your best friend should have borrowed your manual, hit F1 - that usually brings up the help function.
