ARRghh!! whats wrong with his eyes?!!
Ok but seriously, the eyes are bugging me, though it is hard to get photoreference for stuff like this, I think it looks like the eyes are flat, I did a quick paintover (call it what you want) to show you what I mean..

Heheh, yeah and some surgical problems too...
I don't really care if the anatomy is right, but I see that you attempted to put some depth in the picture, and when you see it doesn't work everywhere, it flattens out the whole image. The highlights are a bit too strong for my taste.
And I think you can crop the image to leave only the upper half. You aren't offering anything interesting in the lower part of the image for the viewer to shudder at.
I mean, the thought of having an eye-piercing... hard core!

the eyes look flat because the pin in them makes them come forward, where if they where round it wouldnt, you sould not make the indication of the pins as you did on the skin, you see the skin is pulled tight onto the pins, therefor you can see them bumping out through the skin, but the eyes stay round, you cant see the pins through the surface...
