Others will have suggestions too, and maybe they'll be better ones, but here goes:
Wacom Tablet Properties
On the Mapping/Speed tab, make sure that Pen Mode is checked.
On the Tip Feel tab adjust the Tip Pressure Feel slider and see if that helps.
On the Double-Click tab, uncheck Double-Click Assist box to see if it helps.
In Painter
Go to Edit > Preferences > Brush Tracking and paint a wavy brush stroke using your normal pressure. This will reset your pen pressure and it may help.
Whether or not this has anything to do with your current problem, it's a good thing to know:
Keep your tablet at least 6 to 8 inches (maybe more?) away from your monitor as it can cause the pen to act strangely if it's too close.
Depending on the brush variant you're using, adjustments will vary. Can you tell us which brush variant's you're using when this happens?
When I first got my Intuos 2, I had all kinds of pressure sensitivity problems in some of the several Painter versions I have installed. After much ado, and long talks on the phone, the Wacom techie decided I should delete the wacom.dat file and reboot. The file is regenerated when rebooting. (I understand this file is only in Windows, so I don't know what if any equivalent there is on Macs.) After following his suggestion, everything worked again, pressure sensitivity in Painter Classic, Painter 5.0.3, Painter 6.1, and Painter 7.1.357. Hooray!
