That's pretty weird. Painter Classic shipped with quite a few Wacom tablets as the default pack-in software and had excellent support for the pens. In fact, it's support was far superior to Adobe's implementation in Photoshop.
Is the pen you're using the correct one for the tablet? In the ArtZ series there were a great number of pens --at least seven or so-- and many of them didn't have the same functionality. My first pen didn't have the eraser on the end. My next one only had a single position button instead of the dual button. The one I use now has tilt and angle support --something used in Painter to control the spray direction of the airbrush.
Best advice: make sure your drivers are up to date. If you're using the absolute latest beta drivers, try backing up one or two releases --you might find they're more compatible.

I had the exact same problem with the Painter Classic that came with my new Intuos.
I installed the newest drivers but nothing happened. What worked for me in the end was adding Painter explicitly to the applications list in the tablet properties.
It seems Painter doesn't like being listed with "all other apps"
Hope this'll help...
